Claims FNOL v3 preset, no edits, 2,400 packets. Auto-clear rate: 91.4%. The 8.6% clustered into five patterns: photo-only intake (31%), police report variations (24%), multi-vehicle claims (19%), non-English intake (14%), policy-coverage gaps (12%).
